If so, download the licensed volunteer application. Fill out and sign the application, and kindly attach a copy of your Medical and Driver's License. Please mail the application and license copies to:

Once we receive your completed application, we will submit it to the Health Department for evaluation and you will receive a letter with the status of your application shortly thereafter.

If so, download the non-licensed volunteer application below. Fill out and sign the the complete 7 page application, and kindly attach a copy of your Driver's License. Please note that page 4 MUST be signed and notarized in the appropriate box before mailing. You may also stop by the clinic to have the page notarized if it's more convenient for you.
